The fee for service is your responsibility. If you are NOT covered under a dental insurance policy, payment in full is expected at time of service. If you have dental insurance, your estimated patient portion is due on the date of service. Insurance is not a guarantee of payment.
Any balance over 30 days shall begin to accrue interest at a rate of 1.5% per month. Should your account be placed in the hands of a collection attorney, a 12.5% collection fee will be charged. In addition attorney’s fees of 33.33% of the amount owed, plus court cost, will be added. A $25 fee, plus fees applicable by law, will be charged for any checks that are returned.
